Person who holds a Master's Degree in social work from an accredited school of social work or
A Master's Degree in psychology, gerontology, or counseling from an accredited school and has one year of experience providing social services
in one or more of the fields of aging, health, or long-term care services or
A person who holds a Bachelor's Degree in social work from an accredited school with two or more years of experience providing social services in one or more fields of aging,
health, or long term care services
The ability to effectively work as part of a Multi-Disciplinary Team
Must have Strong Case Management and Communication Skills who is a Patient Advocate
