ABOUT FIVE ACRES' PERMANENCY PROGRAM

Finding Families - Family Finding is a key component to Five Acres' Permanency Initiative, which is designed to establish a lasting connection for every child in its care. Using newly available technologies, Family Finding involves searching, locating, screening and engaging children's family members in their lives.

Adoption Promotion Support Services (APSS) provides support to children and adoptive families to nurture lifetime commitments, ensure permanency for children, expedite the adoption process and reduce disruption of adoption.

Foster Care - Five Acres is an all inclusive agency with open arms to qualified people without discrimination of age, gender, gender expression, sexual orientation, gender identity, religion or ancestry. We recruit, train and certify foster families who have opened their homes and hearts to children. Families receive ongoing training and in-home visits by Five Acres' professional staff to ensure they receive the special support and training they need. We know that fostering and adoption can be a rewarding experience and we want to help you make that difference in the lives of children.

The following positions are currently available in Five Acres' Permanency Program:

GROUP LEADER, APSS -- 20 Hrs. per week (varied hours, including Saturdays & Evenings)
Part-time (20 hours/week) position responsible for facilitation of youth support groups (ages 18 and younger), to ensure quality outcomes, which include permanency for children placed in group care, foster and adoptive homes, and those considering adoptive placement. This position is based out of Five Acres' San Dimas office, and requires scheduling flexibility, including Saturday and evening (until approximately 8:30 p.m.) availability, as necessary to meet the needs of the program.

FOSTER CARE SOCIAL WORKER -- ASL REQUIRED (ON-CALL)
On-call position in Five Acres' Foster Care/Intensive Treatment Foster Care Program, providing social work services to foster children and families, including: Providing foster care placement, planning and implementation for assigned client caseload, and ensuring that children achieve their placement goals in as expeditious a manner as possible. This position is based out of Five Acres' El Monte office, but serves clients in all Five Acres' service areas.
