ABOUT THERAPEUTIC FAMILY SERVICES

Through Five Acres' Therapeutic Family Services Program, family therapy is provided to facilitate successful reunification by aftercare program therapists for children returning to their natural, foster or adoptive families. Therapeutic behavioral services counselors help the child maintain positive behaviors in the home that are essential to maintaining the family placement.

The following positions are currently available in Five Acres' Therapeutic Family Services Program:

THERAPEUTIC BEHAVIORAL SERVICES SPECIALIST (TBS) -- Bilingual Spanish Skills Preferred
Full-time (40 hours/week--weekly schedules vary), non-exempt position providing one-on-one therapeutic TBS services to children and adolescents transitioning from a residential program to a lower level of care, i.e., group homes, foster care, parents, or relatives. The TBS position may include interaction with clients at risk of being placed in a higher level of care. A variety of social services are offered to stabilize clients in a transitioning environment, i.e., therapeutic activities, and role plays and behavioral coaching.

CHILD & FAMILY SPECIALISTS -- WRAPAROUND PROGRAM (ON-CALL) (Bilingual Spanish Preferred) -- Two 2 Openings!
• On-call position (work hours and days may vary) available in Five Acres' Wraparound Program at the El Monte office, responsible for providing appropriate case management and advocacy services for assigned clients in accordance with their treatment plan and needs. This is a billing position. Position responsibilities include, but are not limited to:
• Providing field contacts to assess each client's need for community services and assists client and caretakers in linking up to needed community services in a timely manner while demonstrating family-centered help-giver skills;
• Obtaining and facilitating client information sessions; oversees participation and feedback with regard to service delivery, problem solving and responsiveness to families' identified needs;
• Completing assessments of clients' needs and services to assist in the delivery of service and advocacy;
• Documenting all activities accurately and completely, in accordance with Medi-Cal regulations and within requirements of Wraparound Services which include outcomes for children and families that provide feedback from children and families and community services providers;
• Participating in identifying clients' needs and triggers, within a strength based model, to escalation and barriers; makes recommendations for action to supervisors and treatment team; participates in team/staff conferences regarding solutions to Wraparound Services clients' barriers;
• Collaborating with all levels of management, staff, caretakers, parental/client advisory groups and clients and acts as a liaison between client, agency and community resources, including the incorporation of informal supports, natural family helpers, and other community resources.
