Required Documentations:
1. Valid Driver's License as well as Ambulance Driver's Certification
2. EMT Certificate
3. DMV H-6 Printout (not older than 30 days)
4. CPR & First Aid Certifications
5. DL-51 (obtained from your doctor for health and physical clearance)
6. Must pass a pre-employment Drug and Alcohol test
7. Must be able to be Licensed/Permitted by LADOT
