Job responsibilities:
?Treatment Plans- Develop in conjunction with client, DCFS social workers, and other team members to identify goals and actions to serve the needs and support the strengths of the clients.
?Information, Referral, and Access Services- Advocate and intervene on the clients behalf as well as provide client with in-person and telephone referrals for services.
?Private and Public Benefits- Counsel client regarding the availability of public and private benefits, advise on eligibility requirements, and provide information regarding access to benefits.
?Client Contact & Follow Up- Maintain contact with client and will provide on-going crisis intervention and track clients progress throughout case management interventions and monitor effectiveness.
?Case Conference- Participate in team case conferences
?Training- Attend any training deemed necessary or helpful in relation to work
?Other Activities-Collect client related data and information and information of services provided. Document, compile, and write reports. Attend meetings and supervision related to agency or case management business.
? Other duties as assigned.
Minimum Requirements:
?AA Degree in social services field. Certification in Substance Abuse Counseling a plus.
?At least 1 year in a social service setting with clients that present with multiple and complex problems.
?Bi-lingual, Spanish speaking preferred
?Strong verbal and written communication skills
?Strong organizational skills
?Ability to establish and maintain cooperative working relationships
?Ability to discuss sensitive behavioral issues, including drug use and mental health in a clear and professional manner
?Dependable automobile with insurance, registration and valid California drivers' license.
